Below are a list of online options for contacting us including making requests for Sick notes (Med3)

Fit/Sick Notes (Med 3)
For the first 3 days of illness your employer does not require a sick note.
If you are off sick for up to 7 consecutive calendar days then your employer may ask you to provide a self certification form.
For periods of illness lasting longer than 7 days your employer may ask for a Med3 certificate.
If you feel you can continue working but with adaptations to your role or change in your hours then you may need a fit note.
